		<description>I am a current EMT student, and i found this site very helpful. We were given an assignment to do 4 narratives from 4 scenarios we did in class. I was having so much trouble trying to figure out how to write my narrative, untill i found this site.  I recommended it to my fellow classmates as i hope it will help them as it has helped me. I&#039;ll be posting this to the staff bulletin board- we&#039;ve had some issues with reports (verbal and written) lately among staff. While I use this method, many of my colleagues do not and were never taught about it. Once again, The EMT Spot makes my job a little bit easier :)

I currently use the soap method my guess is that on the assessment part medicaid medicare wants your opinion on why ambulance transport is required.  On the subjective objective plan part I use your method.  Your recomendations would be greatly appreciated in this gray area of documentation.</description>

I&#039;m sure Steve will reply to your question as well, personally I begin typing on the laptop if I have time on the way into the hospital or at least make a few notes if I can. Then as soon as the pram is dressed and the rig is good to run another call I start typing the PCR. Even though SOAP isn&#039;t the same chronologically as CHART, over time you develop the ability to remember your pertinent Hx, pt/bystander statements, and subjective/objective findings and transcribe them where they fall into the SOAP order. Talking w/your pt during transport and the hand off report @ the hospital also cement the info into your head. Like anything else in this job it&#039;s a repetition based skill.
Steve, I&#039;ve applauded this blog many times when you and I talk at work, but why would you transport someone to Aurora South, as listed in your Tx section? I just can&#039;t get my head  around it. I&#039;m still considering the guest blog, it will come in due time and given my current situation in life, it will probably be about a paramedic school subject. Take care.</description>
